this is a common problem and if you search the Mailing list archives you will find info on it. It has also been included in the newest OpenCA guide's FAQ, which I recommend to you to get from the CVS.
 
The bottom line is that Mozilla does not support signing of forms. Your best option is to use the patched Mozilla from wamcom.org. Myself, I tried to use the patched version but I was unsuccessful. I am still hoping for someone to respond to my post and explain if I need to patch something myself or not...

I am using openca 0.9.1-1.  I have created my CA and imported the CA configuration into my RA.  I have also installed the CA certificate in my RA (verified)

however, when i create a new request using the pub interface (on the same server as the ra, the ca is on a stand alone server), and then trying to "approve and sign" the request using the ra interface nothing happens when I click the button.

I am using Mozilla 1.0
