Looking at the apache error log:

Using configuration from
/usr/local/OpenCA/OpenCA/etc/openssl/openssl/Web_Server.conf
entry 38: invalid expiry date
unable to write 'random state'

This means that your index.txt was damaged. Simply check line 38. There is a problem with the expiration date.

If you have only 37 entries then the solution is simple. You have an empty line at the end of the file which is an error for OpenSSL. Remove the last newline. That's all.
