When I follow Michael's guide from February 2003 which explains how to
compile CA and RA parts, I stumble upon Apache documents root
configuration problem :
  ./configure was given --prefix=/srv/ca option
  /srv/ca contains only a "bin" and a "man directories.

So I am unable to set up a right documents root for apache.

I see html files in /usr/local/openca/lib/servers/ca/sheets and also
/usr/local/openca/lib/servers/ra/sheets but don't know which is the
"start" one.

Anyway I don't know where the files for the website part reside and what
the start page is.

I use 0.9.1-3 version and Michael's book seems accurate to this version.
0.9.2 version of the doc has another procedure with automatic
configuration, so doesn't apply here.
