in Phase I,
looks all fine, best is - to remove all installed data and do the install step again, that makes sure, there are no old data laying around from previous installs...
so remove all under /usr/local/openca.0.9... and do make install-... again
i hope this helps and what me confuses a little bit:

my configure prefix
VER=0.9.2-1
PREFIX=/usr/local/openca.${VER}"so - what version are you really using? this is a bit strange combination for me ;o)
