I have had this problem since moving to Apache 2.0 and OpenSSL 0.9.7a. Are you on Apache 1.3 ? If so then this will help me try and chase the bug down as I know that there is the same problem on 1.3 and 2.0. Also, I have tried this on 2 other test systems (both running on the same box as the first) and they only sign 6 certificates (like yours). It does sound like an environment thing to me...
