Try to check Organization fields in request and ca_organization field in
config.xml - they must be the same.
I had working configuration with one extra space after organization name
in ca_organization field - everything was worked, except PKCS#10 and
Serverside Key requests. After deleting this extra space everything
working just fine. I use RC6.

Does the mail Directoty have write privileges for the openca user ?
