You have specified worng credentilas (login data) for the ldap server and are not allowed to write to the ldap...
This is not an OpenCA issue...Check if the data (rootdn) in your slapd.conf and ldap.xml are equal, if you are sure they are, verify if you can connect from the CA maschine to the ldap server by a command line ldap tool
