it looks like you have screwed up some vital files on the filesystem ...

I am not sure but it might be possible that the charset error can be ignored and has nothing to do with your problem - are there other messages in the logs ? (stderr.log)

OpenCA uses some status files beside the databases and I guess you lost one of them. Have you reset the "index.txt" file in var/crypto,

The safest way to re-initalize a OpenCA installation is just to install into an EMPTY directory and copy over the contents of the etc/ directory and rerun configure_etc
