this error can only happen if no Database password is found by
the DBI module. As your DBI.conf seems to be correct and contains
a password, I assume it is not read at all, but possibly some other
file (left over from previous installs?).
Did you receive any errors when you were running configure_etc.sh?
Could you please verify that
$AUTOCONF {"etc_prefix"} = ...
in your openca_start.sh file points to /usr/local/OpenCA/etc/?
