I'm new at this myself but have gone through a couple of dozen install attempts!

First, you mention you used the 'handbook' - is that the 'cookbook' in the 
email list 
archive? The cookbook has fairly complete instructions on setting up a CA and 
RA on a 
single physical machine so I suggest you work with those.
