Actually it is simple - just copy one of the others profile (so you have a
base to start with) and edit it. For Example:

        $ cd PREFIX/etc/openca/openssl/extfile/
        $ cp User.ext Developer.ext

Now edit it - it uses the same options as the openssl config file (if the
name contains spaces, substitute them with '_' in the file name of the
role).

This should work...
