This might be a silly answer, but are you sure you modified the right file?

For example, on my system (based on OpenCA 1.x) if I want to alter the "days" parameter for the Web Server certificate profile I would need to edit /opt/openca/etc/openca/openssl/openssl/Web_Server.conf
There is a separate OpenSSL conf file for each profile.
