I have found the solution for the problem:

Even though original version of "User.pm" didn't have the incorrect constant
I have downloaded OpenCA 1.1.1 Patch for ERR_USER_UNKNOWN at
http://ftp.openca.org/openca-base/fixes/v1.1.1/err_user_unknown/ and
replaced all instances of "initServer" and "User.pm" on source files, them
recompiled and reinstalled.

Shell script "configure_etc.sh" is now updating and renaming all
"*.template" files.
