I have a copy of the MFC4.21 Initialisation/Personalisation commands
reference, and although this is specifically an opencard list, I
hoped that somebody here would be able to help me resolve some
ambiguities that I seem to see...

firstly, regarding intialisation of the code area:

 The diagram on page 2-6 shows that passage from state
INIT_CODE to INIT_FILES can only be achieved by command
INIT_END (possibly requiring INIT_EEPROM first); this command
is described on page 5-11, but says nothing in particular about
what is valid contents for the code area of the card (apparently
C0AD - FFEE for the 16k cards we have, page 1-7). One would
guess that filling this area with zeros would be permissible,
but one cannot be sure. Also not clear if the code linkage 
area (C09F - C0AC) is already initialized or requires
treatment also.

secondly, regarding the necessity for personalisation keys:

 The Personalization Keys supplied in files of type EF_KEY_PERS
are described in the index on page 3-4 of the main MFC commands
guide as "Mandatory during personalization phase". However, it might
appear from the description of the PERSONALIZE command on page
5-14 that one can proceed without its use, but not clear.

Could somebody perhaps supply me with a sample set of commands that
will simply take a freshly-supplied card, and get it into the
personalised state with an MF filling the card which is empty?
