It is not an OCF-related problem. I see it happen often from software that
works directly with PCSC driver (SCardxxx Win32 API functions). I haven't
really examined exactly when it occurs, because reinserting the card
generally fixes the problem - following SCardxxx functions return success. I
think it happens in situations like another program having opened a
connection to the card without closing it, although I would not swear.
