One question:

do i need to start OCF and then stop OCF everytime i read or write on the
card OR Starting OCF only once when program starts and stop OCF only when
card is removed or application is exited??

This might be due to internal errors, such as byte[] alloc/use. This usually
happens when results are not properly built.
Search your applet for bugs.
