You can download the smart card simulator software that accompanies
the book 

        Smart Card Handbook
        W. Rankl & W. Effing
        John Wiley & Sons

from Wiley's ftp site: ftp.wiley.co.uk
go to the directory named /pub/books/rankl and download a file
called rankl.zip. It contains a win32 app that allows you to simulate
the interaction between the smart card and the terminal in which
it is inserted.
