Try getting a version of the pcsc wrapper from Gemplus site. I had the same problem and when I installed that wrapper, it went away.

This occurred after I updated by GemPlus GemPC400 PCMCIA Smart Card Reader
driver from version 2.1.4.0 to version 2.1.6.0.
My OS is Windows XP.
