My guess is that you also have PCSC resource manager running that locks the port. Try running OCF using either OCF-PCSC wrapper or shutdown PCSC resource manager and see if that makes a different.

I know that uninstalling the driver, or removing the reader then plugging it back in works...however, this seems more like a hack, and in win 2k, upon reboot, the drivers are reinstalled, and I need to repeat it again. I know that many people are having the same problem, but this is the only solution I am finding.
