This shouldn't be needed and is possibly why you're getting a blanck screen. In your xorg conf, remove the Modes section and rewrite the Monitor section like that :

From the pci id, the brand is PcChips, but I don't have any crystal ball around to guess the model. It should be pretty obvious if you look at the motherboard itself, the model is most likely written on the board itself.
