Comment(by prl):

 Right, I have done some tests.

 Neither setting Option "XAANoOffscreenPixmaps" or changing depth to 16
 affects the crash. I'll attach xorg.conf / Xorg.0.log for the failures.

 I did manage to tickle another crash while verifying the "Basic" crash
 (i.e. enabling AIGLX without the XAANoOffscreenPixmaps or 16-bit). This is
 '''not''' the "typical" crash, but may shed some light.

 Also, I tried with AIGLX enabled but with a 2nd Xserver (using the "Switch
 User" on the KDE desktop). Since the accel hardware is owned by the first
 user (display 0), the 2nd (display one cannot initialise the hardware and
 so the KDE session works. This seems to confirm that the server dies when
 KDE tries to use the 3D hardware.

 The next step would appear to be extra debugging / instrumentation. Can
 anyone tell me how to do this, or point me towards an openchrome build
 with debug turned up to 11?
