I do know the problem. It comes down to actual errors in the
cross-reference file. Since I will need to build one of these for the
html c-s help, I will try and correct any I find. I've kind of put my
html stuff on the back burner for a little while until I get Adobe
GoLive for OS X and Greg can get the html in as a module in CVS. I'm
working on another little tidbit that I've had users scream they
wanted forever.

When invoking:
Help -> Context-sensitive Help -> <Click on module>
if you click on a Grid module (or a few others), you don't get help on the
module but rather help on some other topic like the data prompter. This is
an old problem I never dug into.
David (or anyone), since you're in the middle of poking around this
area, I thought you might have some insight. Any thoughts?
