Heath is just using this terminology to distinguish between local systems that feed into EHR systems from self-standing EHR/EMR systems which might talk to each other. Actually, the division is not that clear - we use the TDS message to handle CDA documents from some sources (which may be EMR-like) because it works better, and writing a generic transform for CDA level 3 (fully structured) is hard. - thomas beale
