We are involved in a project where we are going to integrate information from
sensors, EHR and PHR and we will use openEHR (and SNOMED CT) for parts of the

Hi, are you aware of any openEHR based personal health record project?
The aim would be to capture wellness type of information integrated with
recent wearable/smart phone based sensor data.
Preferably open source.
