Hi Dave Your use-case is to create data into an EHR. You have different kind of data like problem/diagnosis<http://openehr.org/ckm/#showArchetype_1013.1.169>, medications<http://openehr.org/ckm/#showArchetype_1013.1.1445>, procedures<http://openehr.org/ckm/#showArchetype_1013.1.204>, height<http://openehr.org/ckm/#showArchetype_1013.1.138> and weight<http://openehr.org/ckm/#showArchetype_1013.1.2960>. These are all archetypes in openEHR and I have provided links to them above.
What you need to do is to create a Template based on these archetypes. Based on this Template you can create Compositions. Compositions are documents or transactions containing data into an openEHR system. There are several ways to serialize this data. Both JSON and XML are used in production systems today.
