I got the same bug if i convert a lead into an opportunity while i
created a new partner. Try with a db without demo data and install all
application. (i have just updated so it is with the last revision)

not able to schedule /log a call from an opportunity form view
to test : go to opportunity (sales / sales management / opportunity)
-> schdules /log a call -> click "Schedule a call" after have filled
in the mandatory fields
