OK now I found how to reproduce
set language other than english (german)
open a form with translation field
change English AND German term + click return
user sees correct string
translation record source field is not updated

we work in German language
somehow it is possble to update the name field in the resource without
updating the src field in translation
src fields must always point to a valid source
