In this cases, when a user Edits the document, and then wishes to exit from edit mode without making changes, most of the time he will click "Cancel" instead of "Discard", which *saves the changes* and changes the State to "cancelled"! In the case of Project Issues, it's even worse, since you aren't offered with the option to reopen the issue.
