[sorry, I managed to miss the second part of your mail] When you tell a switch to send packets to the controller, i.e., using output:OFPP_CONTROLLER, they just come up to the FlowVisor encapsulated in the OpenFlow control channel as a packet_in ... exactly the same case as when a packet doesn't match any flow entires comes up to the controller. There shouldn't be anything fundamentally different about this, but at least from your experience, there is likely some low-level bug, either in the FlowVisor or from the switch that you're using.
