The expected behavior should be a packet in, IMHO, but the spec does
not say explicitly so this could be up for debate.  By intuition,
OFPP_TABLE says "do what would have happened if this packet had come
from the normal packet pipeline", so packet_in is the right answer
there.

What happens with a miss on OFPP_TABLE is not configurable, but what
happens on a miss in general is configurable in OF1.1, fwiw.
