I only saw this when I had mixed index (indexed with old opengrok version and new which supports navigation) + obviously there can be bugs in lexers as Jens pointed out, in such case please open a bug and ideally attach the problematic file (if possible, if not and you can figure out what triggers the problem and can send pseudo code it would help too) + sometimes when ctags or opengrok analyzers don't recognize the file and use proper parser you will lack definitions, hence navigation will not work too
