I'm not sure this is a FastStart issue. I had a Polycom HDX 9004 to
test the H.460.18 implementation and it worked fine (in the end) and I
don't think it even tried to do FastStart.

What Simon talks about is that FastStart together with H.460.18
doesn't work in GnuGk 2.3.0, but thats a bug and you should get a "slow
start" and not a failed connection.
