There is an important but small subset of the market where open source
is an absolutely essential feature -- informatics training and research.

When these systems have been deployed in research and academic settings
for a few years and have a track record in the literature, then it is
not just a matter of greater reliability and lower cost, but also
patient outcomes.

Even this is not sufficient because many proprietary health care systems
are able to point to studies and reports from users about their
effectiveness. Commercial exploitation is also needed to turn software
into customer solutions.
