Some quick comparisons: OIO has a browser-based forms editor front-end, FormSIX does not. FormSIX has a "scheme category" look-and-feel selector, OIO does not. OIO automatically stores "data" in a database, FormSIX does not. OIO automatically stores "forms" in a database, FormSIX does not. OIO support Java applet, Javascript, and HTML form elements, FormSIX does not. OIO comes with patients, patient lists, user, and security manager, FormSIX does not. OIO comes with online analysis and reports generation, FormSIX does not. OIO describes forms with XML, FormSix describes forms as DTD. OIO describes data with XML, FormSix also describes data with XML. I think FormSIX is pretty close to what Karsten was asking for. It does not do PDF but seems to be quite flexible.
