This is a very important and interesting question.  It will be interesting 
to see how others respond.

Note that most handheld medical software is free--not open source, but 
free.  Adding open source to free would be a good thing.

The handheld medical software is largely in the category of convenient 
cognitive aid.

Physicians walk around a lot.
