Excellent point.  It would also be helpful to know the source of the
feedback, e.g., Who says "the results are normal" or "the patient
requires a follow-up"?  Additionally, the loginid of the person
providing the feedback should be entered into the database.  This
loginid should be attached to all feedback comments (along with the
date/time).  The loginid, in turn, is listed in the staff database along
with contact information.  This way, if something needs a brief verbal
discussion, you don't have to chase around to find out who provided the
service/feedback, where their office is, what their telephone number,
email address, work schedule, etc. are.  All this is part of standard
project/workflow management.
