I agree, but in this day and age of OTHERS reviewing our work, whether it be 
through Qualdigm, whether it be from insurance companies to decide if they 
want to pay you for what you bill and say you do, the documentation level and 
hassels go up.
Therefore, EMR can not only improve workflow but document better.
This in turn also is more likely than not to protect you in court.

Some patients it is helpful to have prior detailed notes but more often than 
not you don't reread everything. Or even most of it!
