I didn't see this in your earlier email. This almost certainly means
you need to upgrade your HCA firmware. Tavor FW 3.3.1 and Arbel FW
4.6.1 have a bug that will cause this error message; it is fixed in FW
versions 3.3.2/4.6.2.
