If anyone is keeping score, the problem we were having with a Mellanox MT25204 DDR HCA apparently initializing correctly but failing to receive any MAD datagrams from the link is now solved. It turned out to be some interaction between the BIOS and the latest OFED releases. The machine in question is an Intel SE7525GP2 motherboard (7525 chip set). Updating to version P10 of the BIOS firmware fixed the problem.
