Here's an old WFS-T demo :

Look for the 'saveRoad' method inside bdgaRoads.js, you'll see how
values coming from form elements are copied to the feature
('parseFormAttributesToFeature' method) :

Like I said, this stuff is pretty old, but you should be able to
understand how it works and do something similar on your side.
