You're using a compressed proj4js; are you sure it includes both the SRSs you need, 900913 and 4326 ?

The only other item I see is that your mapOptions only specifies the projection and not the whole set of settings. I don't know whether that's the cause, but I usually use this block to get the full Web Mercator Experience:

    projection: new OpenLayers.Projection("EPSG:900913"),
    units: "m",
    maxResolution: 156543.0339,
maxExtent: new OpenLayers.Bounds(-20037508.34, -20037508.34, 20037508.34, 20037508.34)

I guess it's worth a try to paste that in and see if it helps.

Beyond that I think you're doing everything correctly.
