Well the most obvious place to look is the white-listing in the proxy. I
assume you have white-list of available sites - check for typos, port
numbers of the geoserver in that list. Use Firebug to find the call to
the proxy and open it directly in the browser - you can check that it is
expected. Putting some logging in the proxy can also help with getting
to the bottom of proxy issues.
