mdb_info.mi_adxs[] should be a hash table.  It is indexed by
AttributeDescription.ad_index, which can be > MDB_MAXADS.

Currently back-mdb dumps core with 'attributeoptions foo-' in slapd.conf
when ldapadding an ldif which does

        dn: l=e$i,o=adxs
        objectClass: locality
        st;foo-$i: bar

        # optionally:
        dn: l=e$i,o=adxs
        changetype: delete
 
for i = 0...66000.
