After a lot of work from the folks on the IRC channel, especially Paul Fertser, 
Jim Norris, and Matthias Welwarsky, it turns out that the problem was too high 
a value of pullup resistor on the SRST line on our board. It was 100K; putting 
a 10K in parallel with it got it working. Note that this only is a problem with 
some Olimex ARM-USB-OCD (and likely JTAG-TINY) adapters; others work fine, and 
Olimex appears to have made revisions to the pertinent resistors during their 
production runs.

Our board is working, and no changes to OpenOCD itself are needed. Thank you
