I'm working on and off over the holiday period (back at work today actually). I will try to get onsite at the customer whose PBX has this problem, and test your patch. Although the company I work for runs OpenPBX internally, we use a Cisco 1760 with 2 x BRI WICs for PSTN connectivity.
