The tester has reproduced this (with the patch for 691) and shared the logs
(attached).
The campaign has to be assessed first before proceeding further and the ticket
shall be updated accordingly.

Opensaf 4.2 Changeset 3796 is running on 4 node SLES11 64bit cluster
Configured the CLMNA component & its csi's and its associations in the
campaign procedure "safSmfProc=AddCLMNACOMP_CSI" in the campaign.xml, triggered
the middleware
upgrade to Opensaf 4.4 Changeset 4742.
All three operations of adding associations,comp,CSI (in order)were done in
different procInits of
procedure.
Middleware Upgrade was successfully executed and system was up with 4.4FC.
After rollback was initiated, Rolling back of procInit failed during deletion
of "safCsi=CLMNA,safSi=NoRed1,safApp=OpenSAF" failed with return code
rc=SA_AIS_ERR_BAD_OPERATION (20)
