a. The initial cluster is loaded only once, and all machines(nodes) in 
the cluster will sync the information. Any, 
addition/modification/deletion will be reflected to all nodes in the 
cluster.

No, Once one of the controller is loaded, the all the nodes will sync 
from the loaded controller. so, the loading will happen only once in the 
cluster.

immcfg  -f option, is used to update the cluster(after loading) with 
extra information like applications (class and objects).
The updated information will be reflected in all nodes of the cluster.

In testing phase:

Load the cluster with basic xml
update the cluster with application xml (using immcfg -f).

In deployed systems:

Its the user choice, combination xml will be a good option.
