In order to get it to work I had to do the following:
1) built opensc from svn, I used revision 4216 (the latest stable 
release didn't work)
2) use openct-0.6.20
3) libp11-0.2.7
4) engine_pkcs11-0.1.8
5) tell opensc to use openct "reader_drivers = openct;"
6) disabled pcsc, sudo service pcscd stop
7) plug the usb dongle first and then run /etc/init.d/openct start

That's because it does! :-) I just tried openssl-0.9.8k and that works fine.
