i'm using ubuntu 11.10 with openshot 1.4.0 and my JPG file extensions are correct--all end with JPG--and the automatically produced file name pattern is 107_%04d.JPG for files like 107_0210.JPG, 107_0211.JPG

On a separate note which could provide a clue, when right clicking on the Image Sequence Project file and selecting "Preview" the application crashed...
