Yes and Yes.

So you are right, unless someone does an ACTIVE cleanup of a known 
setup, it cannot be done automatically.

A command console level thing to perhaps run to tidy up one or two 
types of setup MIGHT be worthwhile though?  Opensim databases cannot 
grow forever or we will all have trouble.
